Job Accommodation Evaluations

Documentation that helps you do your best work.

A mental health condition should not cost you your job. Under the Americans with Disabilities Act, many people with conditions such as ADHD, anxiety, depression, PTSD or bipolar disorder qualify for reasonable workplace accommodations. We provide job accommodation evaluations in Illinois and clear documentation your employer can act on.

What the evaluation covers

  • A focused assessment: We review your diagnosis, current symptoms and how they affect specific parts of your job.
  • Practical recommendations: Examples include flexible scheduling, a quieter workspace, written instructions, adjusted deadlines or remote work.
  • Clear paperwork: We complete accommodation letters and employer forms in language HR teams understand.
  • Follow-up: If your needs change, we update the documentation with you.

Signs it may be time to reach out

  • Symptoms are affecting your attendance, focus or performance at work
  • Your employer or HR team has asked for medical documentation
  • You are returning to work after leave and need a gradual plan
  • You are unsure what accommodations you could reasonably ask for

Ask our team about current turnaround times when you book, so your paperwork arrives when you need it.

Getting started

Your first visit is a full evaluation with one of our psychiatric nurse practitioners. We listen to your story, review your history and current symptoms, and agree on a plan you understand before you leave. Visits are available in Chicago and by secure telehealth anywhere in Illinois, and follow-up appointments keep the plan on track as things change. You can request an appointment through our online booking page or call our office with any questions first.
